O diagn?stico geralmente ? tardio, sendo adiado devido ? falta de crit?rios padronizados. O Hammersmith Infant Neurological Examination (HINE) ? um instrumento de f?cil aplica??o, baixo custo e com alto valor preditivo para detectar PC, entretanto, ? pouco usado no Brasil, principalmente porque n?o ter sido traduzido, validado e sua confiabilidade testada para os lactentes brasileiros. Objetivo: Traduzir o HINE para o idioma portugu?s do Brasil, e analisar sua confiabilidade inter-examinadores em lactentes brasileiros. M?todo: Estudo metodol?gico, realizado entre junho de 2020 a maio de 2021. A tradu??o seguiu as recomenda??es das diretrizes internacionais. O estudo ocorreu em duas etapas: (1) tradu??o do HINE e (2) an?lise da confiabilidade inter-examinadores da vers?o traduzida, o qual utilizou dois examinadores, que avaliaram os participantes de forma independente. A tradu??o ocorreu em quatro etapas (tradu??o, s?ntese, retrotradu??o e avalia??o pelo comit? de especialistas). A concord?ncia entre os escores foi verificada pelo coeficiente de correla??o intraclasse (CCI), com seus respectivos intervalos de confian?a de 95% e valor de p igual a 0,05. Resultados: Em rela??o a tradu??o as diferen?as encontradas foram resolvidas pela compara??o entre as vers?es anteriores e posteriores realizadas pelo comit? de especialistas, com anu?ncia do autor do instrumento, obtendo a vers?o final do HINE traduzido. A confiabilidade inter-examinadores foi verificada com 25 crian?as (12 pr?-termo e 13 a termo). Excelente confiabilidade inter-examinadores foram encontradas em todas as subescalas do HINE investigadas, como fun??o dos nervos cranianos (CCI2,1 = 0,844), postura (CCI2,1 = 0,852), movimentos (CCI2,1 = 0,830), t?nus (CCI2,1 = 0,894), reflexos e rea??es (CCI2,1 = 0,910) e escore global (CCI2,1 = 0,930). Conclus?o: A vers?o do HINE traduzida para o portugu?s do Brasil mostrou ser adequada e confi?vel aos profissionais de sa?de brasileiros, na detec??o precoce de lactentes com alto risco de PC, al?m de ser uma ferramenta com excelente confiabilidade inter-examinadores em lactentes brasileiros. Introduction: Cerebral palsy (CP) is characterized by a disorder in posture and movement, generated by a non-progressive disorder in the immature brain. Diagnosis is usually late, being delayed due to lack of standardized criteria. The Hammersmith Infant Neurological Examination (HINE) is an easy-to-apply, low-cost, high-predictive instrument to detect CP, however, it is little used in Brazil, mainly because it has not been translated, validated and its reliability tested for Brazilian infants. Objective: To translate the HINE into Brazilian Portuguese and analyze its inter-examiner reliability in Brazilian infants. Method: Methodological study, carried out between June 2020 and May 2021. The translation followed the recommendations of international guidelines. The study took place in two stages: (1) translation of the HINE and (2) analysis of the inter-examiner reliability of the translated version, which used two examiners who independently assess the participants. The translation takes place in four stages (translation, synthesis, back-translation and evaluation by the expert committee). Agreement between scores was verified by the intraclass correlation coefficient (ICC), with their respective 95% confidence requirements and p value equal to 0.05. Results: Regarding translation, the differences found were resolved by comparing the previous and later versions carried out by the expert committee, with the consent of the instrument author, obtaining the final translated HINE version. Inter-examiner reliability was verified with 25 children (12 preterm and 13 full-term). Excellent inter-examiner reliability was found in all investigated HINE subscales, such as cranial nerve function (ICC2.1 = 0.844), posture (ICC2.1 = 0.852), movements (ICC2.1 = 0.830), tone (ICC2,1 = 0.894), reflexes and reactions (ICC2.1 = 0.910) and global score (ICC2.1 = 0.930). Conclusion: The HINE version translated into Brazilian Portuguese proved to be adequate and reliable for Brazilian health professionals in the detection of infants at high risk for CP, in addition to being a tool with excellent inter-examiner reliability in Brazilian infants.
