O exerc?cio da ger?ncia dos servi?os de sa?de da Aten??o B?sica ? carregado de complexidades e, ainda que existam alguns estudos que buscam descrever como a fun??o gerencial ? desempenhada na AB, n?o h? estudos sobre esse tema entre os profissionais que atuam na microrregi?o de Diamantina, MG. Neste estudo buscou-se investigar como tem sido realizada a fun??o gerencial nas Unidades B?sicas de Sa?de (UBS) com Equipe de Sa?de da Fam?lia do Sistema ?nico de Sa?de, na microrregi?o de Diamantina, MG. Trata-se de um estudo misto de delineamento descritivo-explorat?rio, que foi dividido em duas etapas. Na etapa quantitativa foram aplicados question?rios a 53 profissionais que exercem a ger?ncia de servi?os de sa?de com Estrat?gia Sa?de da Fam?lia, na regi?o em estudo, e os dados foram analisados a partir do software STATA11.0. A an?lise dos dados quantitativos revelou que a categoria profissional que assume predominantemente a ger?ncia dos servi?os ? a enfermagem (98,8%), e confirmou as hip?teses de que os profissionais acumulam fun??es gerenciais ?s assistenciais (86,79%) e que os profissionais n?o tem forma??o espec?fica para a atua??o gerencial (73,58%). Os resultados quantitativos demonstraram ainda que os gerentes reconhecem que as atividades propostas para o trabalho gerencial em uma UBS, relacionadas ? gest?o de pessoas, gest?o de materiais, articula??o de rede e participa??o popular, fazem parte da sua rotina di?ria de trabalho. Entretanto, para algumas dessas quest?es, como ? o caso da gest?o de conflitos, da participa??o social e da regula??o da assist?ncia, os profissionais n?o se sentem plenamente aptos a desenvolv?-las. Na etapa qualitativa foram entrevistados, atrav?s de um roteiro semi-estruturado, seis profissionais que acumulam fun??es gerenciais ?s assistenciais e que realizaram curso de forma??o na ?rea gerencial. As entrevistas foram gravadas e transcritas na ?ntegra, tendo sido analisadas de acordo com a an?lise do conte?do proposta por Minayo (2006). A an?lise das entrevistas revelou que a compreens?o de ger?ncia est? atrelada ?s rotinas burocr?ticas, e que o ac?mulo das fun??es gerenciais ?s assistenciais gera a sensa??o de que o trabalho n?o foi desempenhado adequadamente, j? que ambas as fun??es s?o extensas e complexas. As profissionais que acumulam fun??es sentem-se cansadas, sobrecarregadas, desmotivadas e frustradas. Evidenciou-se, ainda, que os cursos de forma??o deram conta de estabelecer a identidade como gerente, de oferecer ferramentas que facilitaram o trabalho, e de ampliar o olhar das profissionais para o usu?rio, enquanto centro do processo de produ??o de cuidado. Conclui-se que, para que a ger?ncia dos servi?os possa representar um fator de transforma??o das pr?ticas, s?o necess?rias algumas medidas: incentivo financeiro aos munic?pios, pelo Minist?rio da Sa?de, que induza a cria??o do cargo de gerente; apoio institucional pela Secretaria de Estado de Sa?de, que garanta uma constru??o do fazer gerencial em conson?ncia com o que ? preconizado pela Pol?tica Nacional de Aten??o B?sica, al?m de espa?os de Educa??o Permanente; cria??o do cargo de gerente da Aten??o B?sica pelas secretarias municipais de sa?de, desvinculado de atividades assistenciais. Regarding health assistance area, the discussion on working process management has been highlighted, in particular what refers to transforming health services in Brazil. Health services management in Primary Health Care (PHC) is full of complexities, and yet some studies have been conducted to describe how the management function is performed in PHC, there are no studies about it among professionals who work in the microregion of Diamantina, Minas Gerais, Brazil. This study investigated how the management function has been performed in Health Centers (HC) which have Family Health Teams, in the microregion of Diamantina. This is a mixed study of descriptive-exploratory design which was divided in two stages. In the quantitative stage, questionnaires were applied to 53 professionals who manage health services with Family Health Strategy, in the region under study. The data were analyzed using the software STATA11.0. Quantitative data analysis show that Nursing is the main professional category which works on the management services (98,8%), and confirmed the assumptions that the professionals accumulate management functions to assistance ones (86,79%) and that the professionals have no specific training to act as managers (73,58%). Quantitative results also demonstrate that managers recognize that the proposed activities for the management work at a HC, related to people management, inputs management, networking and popular participation, is part of their daily routine. However, for some of these issues, such as conflicts management, social participation and assistance regulation, the professionals do not feel fit to perform. In the qualitative stage, interviews were conducted applying a semi-structured script to six professionals who accumulate management functions to assistance ones and had performed a management training course. Interviews were recorded and transcribed in full, and analyzed according to the content analysis proposed by Minayo (2006). Interviews analysis shows that management understanding is linked to bureaucratic routines, and that accumulating management functions to assistance ones bring forth the sensation of poor working performance, since both functions are extensive and complex. Professionals who accumulate functions feel tired, overloaded, unmotivated and frustrated. It became evident that training courses were able to establish an identity as manager, offer tools which facilitate the job, and expand the professionals? view toward the user, as center of the care production process. In conclusion in order that the services management may represent a practice transforming factor, some steps are necessary to be taken: financial incentive to the municipalities by the Health Ministry to induce the manager post creation; institutional support by the Estate Health Department, to ensure the management performance construction in consonance to the recommended in the Primary Health Care National Policy, besides Permanent Education spaces; creation of the primary health manager post by the municipal health department, disassociated to assistance activities. |
