Interprofessional collaboration of nurses and physicians for better patient safety culture

Popis: Izhodišče in namen: Medpoklicno sodelovanje med medicinskimi sestrami in zdravniki je ključnega pomena za zagotavljanje kakovostne zdravstvene oskrbe pacientov, zadovoljstvo pacientov in zadovoljstvo zdravstvenih delavcev. Namen je bil raziskati medpoklicno sodelovanje med medicinskimi sestrami in zdravniki ter kulturo varnosti v ambulantah na primarni ravni zdravstvenega varstva. Raziskovalna metodologija: Uporabljena je bila kvantitativna metodologija, izvedli smo multicentrično presečno opazovalno raziskavo. Za zbiranje podatkov smo uporabili validirana anketna vprašalnika, ki sta bila oblikovana s pomočjo spletnega orodja in posredovana preko elektronske pošte 57 zdravstvenim domovom v Sloveniji. Obdelava podatkov je bila izvedena na podlagi deskriptivne statistike, Spearmanovega koeficienta korelacije in Kruskal-Wallisovega testa. Rezultati: V raziskavi je sodelovalo skupno 210 anketirancev. Ugotovili smo, da je zaznavanje medpoklicnega sodelovanja v ambulantah na primarni ravni pozitivno povezano z zaznavanjem kulture varnosti (rs = 0,440, p < 0,001). Najstarejše medicinske sestre (50 let in več) so kulturo varnosti in medpoklicno sodelovanje pozitivneje ocenile (p < 0,05) kot mlajše medicinske sestre. Prav tako so medicinske sestre z manj delovne dobe (6 do 10 let) kulturo varnosti ocenile z nižjimi povprečnimi vrednostmi (p < 0,05). Pokazala se je tudi pomembnost komunikacije in aktivnega vključevanja pri odločanju o oskrbi za paciente. Diskusija in zaključek: Pozornost je potrebno nameniti mlajšim in manj izkušenim medicinskim sestram, ki sodelujejo z zdravnikom, in se osredotočiti na pomembne dejavnike, kot so komunikacija, prevzemanje odgovornosti in vključevanje k odločanju o zdravstveni oskrbi pacientov, s čimer vplivamo na varnost pacientov. Background and purpose: Interprofessional collaboration between nurses and doctors is key to ensuring quality health care for patients, patient satisfaction and satisfaction of health workers. The purpose was to explore interprofessional collaboration between nurses and doctors and the culture of safety in clinics at the primary level of health care. Research methodology: A quantitative methodology was used, a multicenter cross-sectional observational research was performed. We used validated survey questionnaires, which were created using a web tool and sent via e-mail to 57 health centers in Slovenia for data collection. Data were analysed using descriptive statistics, Spearman's correlation coefficient and the Kruskal-Wallis test. Results: The survey involved 210 respondents. We have found that the perception of interprofessional collaboration in clinics at the primary level is positively related to the perception of a safety culture (rs = 0.440, p < 0.001). The oldest nurses (50 years and over) more positively evaluated safety culture and interpersonal cooperation (p < 0.05), compared to younger nurses where the grades were lower. Also, nurses with less working years (6 to 10 years) have perceived safety culture as poor (p < 0.05). The importance of communication and active involvement in patient care decision-making has also been demonstrated. Discussion and conclusion: Attention should be paid to younger and less experienced nurses which work with a doctor and focus on important factors such as communication, responsibility and involvement in decision-making about patient care that affect patient safety.
