Hospitals need to join forces to better coordinate and integrate patient care across hospital boundaries and enhance task distribution. Three types of collaboration are currently regulated by the Belgian Hospital Act (association, group and merger). Yet, these legal collaboration forms are no longer sufficient. A number of shortcomings in the collaborations and corresponding governance structures could be identified. The barriers mainly apply to hospital associations and hospital groups. Among others, the decision-making process is time-consuming and complex. This is not only due to the legal composition of the committees at the level of cooperation and the need for feedback at the level of individual hospitals, but it is also a consequence of how care is organized to date. Some of the outlined obstacles cannot be solved by the mere establishment of networks. Some barriers will require adaptation of legislation, other barriers require thorough reform in the hospital sector itself.
