Hemipelvectomy due to explosion: possibility of occupational rehabilitation

Popis: U radu se obraduje klinički slučaj 21 godišnjeg mladića, pripadnika Hrvatske vojske, koji je zadobio kompletnu avulziju desne natkoljenice, desne polovice zdjelice, vanjskog spolovila te potpuno prekinuće vanjske ilijakalne arterije i vene prilikom eksplozije protutenkovske granate u vojarnici pri neopreznu rukovanju. U svijetu postoji malo objavljenih prikaza pacijenata koji su preživjeli traumatsku hemipelvektomiju. Ova ozljeda opterećena je izrazito visokom smrtnošću (60-100%). U opisanom slučaju nakon brzog kirurškog zbrinjavanja postoperativno se razvila anaerobna klostridijalna infekcija. Intenzivnom terapijom i fizikalnom terapijom pacijent je osposobljen za samostalan život uz kretanje u invalidskim kolicima i hodanje sa štakama i s protezom. Uključen je u organizirani program osposobljavanja za rad na računalu. Nastavljeno je s fizikalnom terapijom, a nužna je stalna psihoterapija zbog velike invalidnosti. Preostaje obveza uključivanja u rad odgovarajućim ergonomskim pristupom. U tu svrhu nužna je suradnja stručnog zdravstvenog osoblja s Ministarstvom obrane, Ministarstvom rada i socijalne skrbi, Republičkim fondom mirovinskog osiguranja i Ministarstvom prosvjete.
A case of traumatic hemipelvectomy due to explosion of an antitank shell in a 21-year-old Croatian Army soldier is described. Explosion was due to inadvertent handling ot the shell in the army barracks. Literature data on persons who survived traumatic hemipelveclomy are extremely scarce; the injury is characteiized by a very high death rale (60-100%). After prompt surgical treatment the patient developed a postoperative anaerobic clostridial infection. Owing to intensive clinical and physical therapy the patient was enabled for moving around in a special wheel-chair and walking with the help of crutches and a prosthesis. He was also enrolled in a training course in computer use. Continual physical therapy and psychotherapy were essential because of a high degree of disablement. Making the patient self-sufficient, fit for work and finding him an appropriate job is considered to be the responsibility not only of the professional medical team but also of joint efforts on the pari of competent state ministries, Ministry of Defence, Ministry of Labour and Social Welfare, State Pension and Insurance Fund and Ministry of Education, Culture and Sports.
