RA患者の歩行訓練のための平行棒把持スプリントの適用

Popis: Rheumatoid arthritis patients have multiple joint ploblems, often making them difficult to use gait aids after joint replacements in lower extremity. The purpose of this study was to investigate a influence of dysfunctional upper extremity in the patient with rheumatoid arthritis and application of hand splint during gait training after joint replacements in lower extremity. This splint was designed by us for rheumatoid arthritis patients to assist them to walk in parallel bars after surgery. Fourteen patients suffering with rheumatoid arthritis who has undergone hip or knee joint replacements were selected for the subjects of this study. They were divided into three groups.The first group:five patients could not walk in parallel bars without hand splint. The second groups: six could not walk without the aid of splint. And the third group: three could not walk in spite of the aid of splint. We evaluated grasping ability, range of motion, joint pain and muscle strength of three groups. Then we found some difference among three groups. The first group painents could hold bars, because their finger and wrist joint did not have any pain which had mild limitation in range of motion. The second and the third group patients had severe pain in their wrist joints. And their wrist joints had also severe limitation in range of motion. The third group patients had severe limitaion in range of motion in their shoulder and elbow joints. They had multiple joints pain in upper extremity. Muscle strength of wrist of wrist of the first group patients were good, but that of orther two groups patients were fair. Muscle strength of shoulder and elbow joint of the third group patients were fair, but that of orther two groups patients were good. These results were indicated that certain muscle strength is necessary for application of hand splint during gait training after joint replacements in lower extremity for rhematoid arthritis patients.
