A clinical study of surgical treatment for diverticular disease of the colon

Popis: | We have reviewed 30 patients (22 men and 8 women with a mean age of 54.5 years old) of colonic diverticular disease who had undergone surgical treatment in the last 22 years. The major clinical symptom was abdominal pain, 19 cases with local peritonitis and 9 with melena. Of the 30 cases, 18 patients had right colonic, 10 had left colonic, and 2 had bilateral colonic diverticula. Preoperative diagnoses were 6 cases of acute appendicitis, 3 recurrent diverticuhtis, 2 diverticular perforation, 8 intestinal bleeding, 1 colovesical fistula, 5 intestinal stenosis, and 5 cases of intra-abdominal abscess with a palpable mass. The surgical procedures performed were ileocecal resection in 9 cases, partial resection of the ascending colon in 1, right hemicolectomy in 9, sigmoidectomy in 7, and left hemicolectomy in 1 case. Primary anastomosis was performed in all the cases. Hartman n's procedure was used in two cases. Right-sided diverticular disease was associated with a high incidence of diverticuhtis and bleeding, while the left-sided type with stenosis. In the bilateral type, bleeding was observed. Postoperative complications were 2 cases of wound infection and 1 case each of intra-abdominal abscess, intestinal obstruction, and congestive heart failure. All cases were treated conservatively. So far recurrence of diverticular disease has not been observed in any of our cases. Complications of diverticular disease of the colon vary widely in this study, therefore an adequate diagnosis and surgery should be done in accordance with the patient s general condition. 論文 |
