Lifestyle and Associated Factors of Hypertensive Patients Identified Through the Annual Health Screening Examination

Popis: 本研究の目的は、基本健康診査の結果、「血圧」要医療と判定された者の生活習慣とHBMに基づく高血圧に対する認識等の関連要因を明らかにすることである。U市の平成8年度基本健康診査受診者で「血圧」要医療と判定された314人に訪問面接調査を行った。主な内容は、生活習慣、高血圧に関する認識、健康に関する情報への関心、市の保健事業への参加、基本属性である。回収率は81.4% (256人) で、その中で分析対象者は定期受診し、自己申告による服薬治療の指示のあった201人である。(1)本研究の対象者は、禁煙、節酒、運動、減塩の実施率は、他の報告と比較して高率であったが、休養は低率であった。(2)好ましい生活習慣の実施項目数を上位群、下位群に分けて分析を行った結果、収入の伴う職業有りの割合は下位群が上位群と比較して高かった。(3)健康情報への関心度は、テレビの健康番組を見る、新聞の健康記事を読む割合は上位群が下位群と比べて高かった。市の保健事業への参加では、基本健康診査を毎年受診する割合は上位群が下位群と比べて高かった。(4)高血圧に関する認識では下位群の者は高血圧に関する保健行動を実行する上での障害の認識得点が高かった。以上のことから、高血圧要医療者の生活習慣実施の関連要因として、収入の伴う職業あり、テレビ、新聞の健康情報への関心、基本健康診査を毎年受診する、高血圧に関する保健行動を実行する上での障害の認識が確認された。今後は、高血圧に関する認識を加味した保健指導の必要性ならびに基本健康診査未受診者に対して積極的にアプローチすることの重要性が示唆された。
The purpose of this study is to investigate the lifestyle and associated factors of hypertensive patients using the Health Belief Model. Three hundred and fourteen persons, who were diagnosed as hypertensive by the annual health screening examination of U City were contacted and 256 persons(81.4%)agreed to participate in the study. Of these, 201 subjects undergoing treatment were eventually chosen as participants. Subjects were interviewed in their own homes by trained interviewers. The content of the interviews consisted of questions that explored lifestyle change, the subjects' awareness and knowledge with regards to hypertension, interest in healthrelated information and subjects' participation in city-sponsored health services. (1)Total percentage of persons adhering to "no smoking" "moderating drinking" "getting enough exercise" "decreasing salt intake" health practices was high when compared to other research findings. However "getting proper rest" was low when compared to other research findings.(2)Subjects were then classified into two groups(high and low)according to their score on a health practices index(HPI). It was revealed that persons working were comparatively higher in the high HPI group.(3)Further, interest in health related information through television programs and newspapers was higher in the high HPI group. Participants in city-sponsored health services were comparatively higher in the high HPI group.(4)Low HPI groups, with regard to perceived barriers to preventive action scored higher than high HPI groups. Henceforth, associated factors are persons working, interest in health related information through television programs and newspapers, participating in city-sponsored health services, and regard to perceived barriers to preventive action. It is necessary that subjects awareness of hypertension be heightened through health education programs. It is also important that persons not taking part in health screening examination explanation meeting be actively approached.
