Regional health issues perceived by elderly residents of a small isolated island and their solutions : Regional assessment utilizing residents' viewpoints at the practice level

Popis: | 目的:実践レベルでの住民の視点を活用した地域アセスメントを検討するため、コミュニティアズパートナーモデル(以下、モデルと略)を用いて、小離島の高齢者の「認識」から、地域の捉え方、地域の強み、地域の健康課題とその解決方法を導き出すことを目的とする。方法:研究協力者は、A村保健師から紹介を得て了解の得られた高齢者6名である。方法は、モデルの枠組みの住民の「認識」を用いてインタビューを行い、質的帰納的に分析した。結果:1.地域の捉え方及び地域の強みは、モデルの「地域のコア」及び「サブシステム」 の構成要素を包含していた。特に、【価値と信念】についての語りでは、(困難を乗り越えることができる島)、(人材の育つ島)などと捉え、既存資料や統計資料などの客観的データでは把握しづらい内容であった。2. 地域の健康課題は、高齢者ケアや介護問題、離島医療、若者の健康問題、つながりの弱まりを挙げていた。その解決方法は、「住民としてのセルフケアと支え合い」と「専門職・行政への期待」があった。結論:住民の「認識」から、地域の捉え方と地域の強みは地域アセスメントに活用が可能である。そして住民は、健康課題の解決方法として、専門職・行政との協働による取組みを希望していた。 Aim : The aim of the study was to consider a regional assessment approach that utilizes local residents' viewpoints at the practice level. Toward this end the Community as Partner Model was used to investigate perceptions of the local area. its strengths, health issues. and their solutions from the viewpoint of elderlyresidents of a small isolated island. Methods : Participants were six elderly residents who gave written informed consent to participate after receiving an explanation of the study from a health nurse in village A. Interviews were conducted with the participants focusing on their 'perceptions' within the framework of the model. Data were analyzed by the qualitative inductive method. Results : (1) Participants' perceptions of their regional area and its strengths contained the constituents of 'regional core' and 'subsystem' in the model. Particularly in relation to 'value and belief' , they described their island as 'the place where difficulties can be overcome' and 'where human resources are nurtured' . The findings of the interviews went beyond the objective data gathered from previous studies and statistical data. (2) Concerning local health issues, the participants raised problems such as those relating to care for the elderly, nursing, medicine on isolated islands, young people' s health, and weakening of relationships. As for the solutions to the issues, 'self-care and mutual support of residents' and 'expectations for professionals and administrative offices were identified. Conclusion : Examination of local residents' viewpoints revealed the possibility of utilizing the perceptions of a local area and its strengths in regional assessments. Regarding solutions to health issues, residents were found to have a desire to work in collaboration with professionals and administrative offices. |
