The Characteristic of Student's Learning by Difference of Practical Training Institution : Focus on Subject Understanding

研究目的は,A大学の4年次精神看護学実習において,病棟実習をおこなった3グループと,社会復帰施設で実習をおこなった3グループの各実習施設における対象理解に焦点をあて,学生の学びの特徴を明らかにすることである.分析対象は,6グループ(病棟3グループ,施設3グループ)46名が実習後に提出したレポートの内容である.分析方法は,A大学で掲げる対象理解についての実習目標9項目に沿って,レポートの記述を実習施設別に抽出し,内容の類似するものをまとめてサブカテゴリとし,さらにカテゴリ化した.記述の抽出,カテゴリ化では,研究者間で繰り返し検討し,信用性の確保に努めた.分析の結果,両方の実習施設に共通するものとそれぞれの実習施設の特徴について分析した.受け持ち患者の特徴は,病棟はうつ病が最も多く,成人期と老年期の回復期の患者が大部分であった.社会復帰施設は統合失調症がほとんどを占め,成人期の長期在宅生活者が大部分であった.分析の結果,病棟実習では疾患・治療などの視点や日常生活行動面の視点からの対象理解が詳細であった.社会復帰施設では,生活者としての対象理解や精神障害者の抱える偏見などの視点が詳細であった.これらの視点はともに重要であり,学生がそれらの学びも得られるように実習施設に応じて不足しがちな視点に学生が目を向けられるような指導者の学習支援が必要であると考えられた.さらに,学びの多い実習とするための実習形態を模索していく必要があることが示唆された.
The purpose of this study is to compare three groups which received practical training at a rehabilitation institution with three groups which received practical training in a psychiatric ward, and to clarify the characteristics regarding the students' ability to learn and understand the patients' needs at each training institution. The analysis was based on the data obtained from 46 reports that were submitted by the students after the training. A data analysis was performed regarding the students' understanding of nine target training items. In the ward, the patients mostly consisted of cases of depression or dementia. At the rehabilitation institution, most patients had schizophrenia; and these patients had also lived at their homes for a long time. As a result of the analysis, in ward training, most students came to understand the patients from the viewpoint of their illness, the therapy as well as the patients' normal actions and behavior in everyday life which are affected by their symptoms and illnesses. At the rehabilitation institution, the students came to understand the patients as real, live human beings and also came to comprehend some of the various prejudices that such mentally handicapped people encounter. It is therefore considered to be important that the students learn the different viewpoint of the patients needs at both institutions. It is also important for the teachers to help the students understand these important aspects of nursing. In addition, the development of new training methods and devices is also necessary in order to improve the training of such students.
