外来患者の主体的な受療継続を支援する専門性の高い外来看護師の実践

Popis: | 外来では、慢性疾患患者に対し療養相談などの外来看護が機能していない現状があり、それを変えていくためには、一般の外来看護師の意識変革が必要である。そこで本研究は、専門性の高い外来看護師が慢性疾患患者の主体的な受療継続を支援するためにどのような考えをもち実践しているのかを明らかにし、一般の外来看護師が行う支援の示唆を得ることを目的とした。CNSおよび外来プライマリー制で外来看護を実施している看護師計3名に対し、半構成的面接を行いデータを質的記述的に分析した。その結果、47のサブカテゴリー、11のカテゴリーが抽出された。さらに、患者の人生にずっとつきあう決心と責任を持って支え続けるという【外来看護師の役割意識】を基盤として、患者が【“受診してよかった”と思えるように支援する】【患者を生活の視点で捉えて関わる】【受療継続が困難な患者にはあらかじめ把握して関わる】【患者をありのまま受け入れて支援する】という5つの中核カテゴリーが明らかになった。一般外来看護師が行う主体的な受療継続の支援に示唆が得られた。 Currently, outpatient nurses are not tasked with providing medical consultations and other services at outpatient departments to patients with chronic diseases. For this to change, a shift in awareness among regular outpatient nurses is needed. Therefore, this study aimed to obtain suggestions for implementation of nurse-based support for regular outpatients by elucidating what kind of mindset highly specialized outpatient nurses adopt in practice to provide continuous support in managing the lives and treatment of patients with chronic diseases. Semi-structured interviews were conducted with a total of three nurses who practiced outpatient nursing as certified nurse specialists and as part of outpatient primary nursing care. Data were then qualitatively and inductively analyzed. A total of 11 categories and 47 subcategories were extracted from the interviews. Analysis also revealed five core categories comprised of “Role awareness of outpatient nurses,” which represented nurses' resolve and duty to continue supporting a patient throughout their life, and “Support that makes patients glad they sought a consultation,” “Understanding and interacting with patients from the standpoint of the patient's life,” “Familiarizing oneself with patients who struggle to continue managing their lives and treatment before interacting with them” and “Supporting patients with an acceptance of who they are,” which were based on the first core category.
