In search of diet and exercise routine : Triggered by the joy found in diabetes treatment

Popis: | Persons with type 2 diabetes need to exercise self-control, which is often difficult as they must deal with many things. The author became acquainted with a research participant who gave full attention to a self-regimen of meals and exercise to lose weight. The purpose of this paper is to carry out year-long fieldwork and non-structured interviews to relate how his experience was formed. Hospitalization made a drastic change in his diet and exercise routine enabling him to walk with ease. He was immersed in the joy of “I can do it.” Setting a goal to expand his potential, he practiced the diet and exercise that had changed him. Although he overdid it after discharge and had to review his diet pattern when this was pointed out by physicians, he tried diversifying his meals. Carefully monitoring his weight, he earnestly tried to achieve safe weight reduction. For a year he endeavored to find the appropriate way of eating and exercising and felt rewarded as his system worked well, which was reflected in his weight. Referring to Merleau-Ponty, the author noted that his devotion to his routine started when he found significance in the joy of “I can do it” and the goals. The outcome of his search for a better routine, which accounts for a large part of his life, was confirmed by his lived experience in the immediate appearance to measurement values including weight, which determined his next step. When the diabetic appear to be following treatment requirements, they may actually be making efforts for other reasons that are more important to them. This gives rise to the possibility of new diabetic care that would see medical professionals watch over a person with type 2 diabetes while allowing them to practice their desired living patterns to the extent that it would not cause a deterioration in their condition. 2型糖尿病者は多様な生活のなかで自己管理を継続しなければならないが,その実践に苦慮することも多い。そのなかで,筆者は自分で決めた食事・運動に専念して減量し続けた研究参加者と出会った。本稿の目的は,その研究参加者の経験の成り立ちを,1年間のフィールドワークと非構造化インタビューを介して記述することである。入院によって食事・運動の仕方が大きく変わったその研究参加者は楽に歩けるようになり,“できる”よろこびに浸った。彼は,その身体をもたらした日々の食事・運動実践を志向し続け,さらなる可能性の拡大を目標にしていった。退院後に主治医たちからやり過ぎを指摘されて食事パターンを見直すことになったが,次第に食の自由を広げつつ,体重増加を判断基準にして安全な減量を成し遂げる探究的な行為に専心していった。彼は,1年間にわたり食事・運動実践のうまいやり方を探究し続け,体重に成果が出るそれらの実践のシステムがうまく作動することに生きがいを見いだして暮らしていた。メルロ=ポンティの思想を参照しつつ,その研究参加者の食事・運動実践への没頭は,彼の身体が治療を契機に始めたそれらの実践を“できる”よろこびや目標として意味づけるようになったことから発生していたこと,毎日の大半を占める日常的な実践のうまいやり方の探究は,生きられた体重によってその成果が確かめられ,次の実践が選択されていたことを考察した。2 型糖尿病者が治療実践をしているように見えても,その人にとっての動機づけにより治療とは別の取り組みをしている可能性があり,病状悪化に至らぬ範囲内で病者の望む実践を見守るという新たな糖尿病医療の可能性が示唆された。 |
