Comparison of Cu-62-ATSM and C-11-Methionine in cancer of the Cervix treated by radiation therapy

Popis: Purpose:To determine if Cu-62 labeled diacetyl-bis (N (4)-methylthiosemicarbazone); (Cu-62-ATSM) imaging of tumor hypoxia is associated with C-11-methionine imaging of amino acid metabolism in cervical cancer. The carbon ion radiotherapy (CIRT) has been introduced in our institute, and we have treated cervical cancer using carbon ion beam or intracavitary treatment and whole pelvis irradiation.Cu-62-ATSM has been used clinically as a positron emission tomography (PET) tracer for the delineation of hypoxia. Tumor hypoxia is a critical factor in irradiation of malignant tumor. Method: PET/CT was performed in eighteen patients with cervical cancer for evaluation of both tumor hypoxia using Cu-62-ATSM and amino acid metabolism using C-11-Methionine. Histologically confirmed ten squamous cell carcinoma, five adenocarcima, two Adenosquamous carcinoma and one Malignant melanoma were enrolled in this study. PET/CT studies were underwent before and after treatment. Tumor uptake of each tracer was analyzed semi-quantitatively by Tumor-to-Normal-Tissue-Ratio (TNR). Result: Pretherapeutic mean TNR was 8.7 in C-11-Methionine and 2.9 in Cu-62-ATSM, and thus image contrast between tumor and surrounding normal tissue was higher in C-11-Methionine PET/CT image than in Cu-62-ATSM PET/CT image. Pretherapeutic TNR of both Cu-62-ATSM and C-11-Methionine showed significant difference between squamous cell carcinoma and other tumor group (p=0.0165, p=0.0060, respectively). Posttherapeutic mean TNR was 2.9 in C-11-Methionine and 2.1 in Cu-62-ATSM. Posttherapeutic TNR of either Cu-62-ATSM and C-11-Methionine showed no significant difference between squamous cell carcinoma and other tumor group, respectively.
