1) The patient was a 44-year-old man who had a la r g e physique and was markedty obese. He had no family history of gout. 2) For the past 6 years, he ex p erienced recurrent attack of gout and showed gouty nodules (tophi) in the left ear, left elbow, both insteps, and inner aspect of the left foot. Since about a month ago, be has had severe attack a pain in the left flank abdomen and discharged a stone of azuki bean large. 3) Serum uric acid was 8.5 6 mg/dI (Folins method). Urate was demonstrated in the gouty nodules and the discharged stone (the murexid reaction was+ + +). 4) With administration of phenylbutazone, an improvement of the s y mptoms and lowering of the serum uric acid level were achieved. |
