Popis: | The porcine aortic xenograft bioprosthesis was studied by ultrasound in vitro during simulated aortic placement with pulsatile flow. A smooth continuous surrounding structure allowed thorough echographic examination, whereas a highly sound-reflective (pleated) surrounding structure prohibited echographic examination. The valve leaflet movement was similar to that in the normal human aortic valve. The supporting bioprosthetic stent appeared as two thick parallel lines on either side of the leaflets. Inability to record the xenograft leaflets and their movements in all patients was attributed to an adverse plane of the aortic annulus or to surgical insertion of the bioprosthesis with a commissural support between the anterior aortic wall and the leaflets. To improve the opportunity for echocardiographic examination, which is the best noninvasive method for following up the effectiveness of bioprostheses, we recommend surgical insertion of the bioprosthesis with the largest (right coronary) cusp overlying the interventricular septum. |
