Measles in Mbeya, Tanzania--1981-1983

Popis: In the wet season of 1980-81 a severe measles epidemic swept the southern highlands region of Tanzania. A prospective study of measles cases admitted to the government hospital was started to find out the age at which the children contracted measles the vaccination state and the complications leading to death. A total of 913 cases were admitted from September 1 1981 to August 31 1983. 83% were discharged better or transferred to the nutrition section of the childrens ward 9% absconded and 8% died in hospital. The greatest number of admissions for any 1 year was in the 1st year of life (26%) and in this group the commonest age for admission was 8 and 9 months. In 41% of the cases the vaccination status was not known. Of the remaining cases 65% were unvaccinated and 35% vaccinated. Conclusions from this study point to a high measles morbidity and mortality in children under 1 year of age in this region of Tanzania. This suggests that immunization against measles during the 9th month of life is too late. Respiratory complications are still the main cause of death but gastroenteritis plays a large part in 1/4 of those who die. There was also a high mortality rate in 3rd degree malnutrition and in measles acquired in hospital. Finally immunization during the incubation period of measles associated with 3rd degree malnutrition carries a particularly poor prognosis.
