Popis: | T HE automobile accident is the single greatest cause of closed chest injury, and it cai:ries the highest mortality rate." Although the bony thorax and the shoulder girdle offer partial protection to the thoracic viscera, they are ineffective against a penetrating injury or the sudden deceleration experienced in an automobile collision. A patient who has sustained rib fractures should be examined for possible injury to the pleura, lungs, or blood vessels, while the thoracic cage itself takes a secondary place in management. In the occasional patient whn breaks a rib or two during a severe coughing episode, injury to internal structures is minimal. Fracture of the upper three ribs may signal more serious injury to the tracheobronchial tree and the vascular structures. ',"'~a Injury to the lower three ribs may be associated with laceration of the fiver, spleen, or kidney. In childhood, the ribs are resilient, but they become more rigid and brittle with advancing age, so that the incidence of rib fracture increases after the third decade. ~ Pathologic fractures and trauma resulting from penetrating injuries will not be discussed here because of space limitation. |
