| Popis: | Care homes in England deliver complex packages of care to people with long-term conditions. The current policy focus on improving care pathways, choice and self-determination for people with long-term conditions offers tremendous opportunities for partnership working between the private and the public sector. This article describes an initiative to introduce the role of the community matron into the care home setting. |
