Intrathoracic parathyroid adenoma

Popis: A 43-year-old woman was referred to University Hospital for evaluation of hypercalcemia. She had a history of renal calculi and hypertension. Her serum calcium and phosphorus levels were 10.9 mg/dL and 2.5 mg/dL, respectively. Her potassium level was 4.6 mEq/L. Urinary calcium (24 hours specimen) was 234 mg/dL (normal). A Tc-99m sestamibi scan was performed (Fig 1), which revealed a mass in the mediastinum (arrowhead). Serum chloride to phosphorus ratio was 42.5. Intact parathormone level was 150 pg/mL (normal range 8 to 97 pg/mL). The patient underwent mediastinal exploration via a median sternotomy. A 3.5 3.5 1 cm mass, weighing 10 g, suggestive of an ectopic parathyroid adenoma, was identified and excised from the anterior mediastinum (Fig 2). The vascular pedicle of the mass (arrow) originated in the neck. The patient was discharged on the second postoperative day and has remained asymptomatic. A serum calcium level 1 year after surgery was 8.3 mg/dL.
