Popis:	Electrical alternation of a single electrocardiographic wave is rare and has not been adequately explained. We describe a case of U wave alternans associated with extreme ventricular irritability. Relationship to hypomagnesemia is indicated by rapid response to magnesium therapy. Mechanisms that may be responsible for this phenomenon are discussed.
