Nutritional status features in juvenile rheumatoid arthritis related to vitamin D status

Popis: | Objective. To evaluate the nutritional status (NS) in children with juvenile rheumatoid arthritis, taking into account vitamin D status, activity and duration of the disease. Material and methods. Twenty'seven children with juvenile rheumatoid arthritis aged from 2 to 17 years and 20 healthy children of the control group were examined. Body mass index and the nutritional status ratio were calculated for the NS evaluation. The concentration of plasma leptin was determined by solid-phase radioimmune assay, using Belgian sets on the gamma counter STRATES. The 25(OH)D level was detected using an immunoassay kit for measuring 25-hydroxy-vitamin D and other hydroxylated metabolites in serum or plasma manufactured by Immunodiagnostic Systems Ltd (IDS Ltd) (UK). Results. The nutritional status violation of different degree and the decreased vitamin D concentration were observed practically in all patients with juvenile rheumatoid arthritis. A sufficiently high direct correlation between the severity of malnutrition and the degree of activity of juvenile rheumatoid arthritis, low leptin and of vitamin D serum levels were found (r=0.60). Children who suffered for a long time from juvenile rheumatoid arthritis (more than two years) had a less pronounced malnutrition compared to the newly diagnosed children, but more pronounced vitamin D deficiency. Conclusions. Monitoring of 25(OH)D and leptin serum levels is indicated for evaluation of malnutrition and its further correction in children with juvenile rheumatoid arthritis. |
