Insulin independence after allogeneic intraportal islet transplantation: relation to functional tests

Popis: INSULIN independency in insulin-dependent diabetes mellitus (IDDM) patients after allogeneic islet transplantation has been achieved on several occasions. The International Islet Transplant Registry reports 8 cases, in which insulin could be discontinued for more than one year. However, recent data suggest that the success rate has increased. Several factors are involved in the process of insufficient insulin secretion: chronic rejection, recurrency of diabetes, impaired viability, and possibly beta-cell dysfunction. In a number of cases, a fairly constant level of C-peptide can be demonstrated in the serum for several years even though insulin is needed for satisfactory correction of blood glucose. The reason for this observation is not clear although, in case of a coincident renal transplant, a reduced kidney function may account for a lesser clearance of C-peptide and thus false high levels. We report here on the functional outcome of one patient followed for 2.5 years after allogeneic islet transplantation.
