ENDODONTIC PERIAPICAL LESIONS ARE CHARACTERIZED WITH THE HIGH PREVALENCE OF VIABLE AND ACTIVEMICROORGANISMS

Popis: Summary Subjects or Study Selection : This investigation was performed according to Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) statement guidelines and registered at PROSPERO. An electronic search was undertaken, with no date or language restrictions for studies published up to September 2019 to June 2020, in the five electronic databases (i.e., PubMed, Embase, Scopus, Web of Science and the Cochrane Library) The search strategies were carried out using keywords and medical subject heading (MeSH) terms. Moreover, the authors reported the use of additional literature sources (e.g. citation mining and/or gray literature). The electronic search process was made by one experienced scientist, while two reviewers independently performed the selection of potentially eligible studies following a two-phase process (i.e. screening and eligibility phases). In the case of a disagreement a third reviewer was consulted. Based on eligibility criteria, 23 studies were included for the qualitative and quantitative analyses. Key Study Factor : The authors performed a systematic review and meta-analysis of cross-sectional studies on the prevalence of different microorganisms and their viability/activity in AP. Included, cross-sectional studies covered populations consisting of patients with permanent teeth with secondary/persistent endodontic infection and the presence of AP verified using periapical radiographs, panoramic radiographs or cone-beam computed tomography (CBCT) images. Main Outcome Measure : The primary outcome of this investigation was the prevalence of different microorganisms in AP. Secondary outcomes were related to the subgroup analyses (e.g. identification methods (culture and molecular), clinical presentation (symptomatic and non-symptomatic), period of publication ( Main Results : Based on eligibility criteria 23 cross-sectional studies were included in the final review. The authors performed a meta-analysis of pooled data to investigate primary and secondary outcomes. Briefly, three studies were appraised as low risk of bias, 16 as moderate, and four as high. Conclusions : Apical periodontitis is characterized with the high prevalence of viable and active microorganisms. There is a significant difference in the prevalence of microorganisms within different geographic areas. The genera Actinomyces, Fusobacterium and Prevotella, and Herpesviridae family were the most prevalent taxa in investigated samples.
