Herein, we report two rare cases in whom pain symptoms started during the last trimester of pregnancy. Both patients had similar clinical presentation; however, a meticulous analyses helped us diagnose pregnancy-associated inflammatory sacroiliitis in the first case, and sacral stress fracture in the second case.
