A 55-year-old man with erythematous urticarial plaques, hepatosplenomegaly, and bicytopenia

Popis: 55-year-old man presented with the complaint of abdominal fullness. Abdominal ultrasonography revealed hepatosplenomegaly. Laboratory tests showed leukopenia and thrombocytopenia. His past medical history was significant for erythematous urticarial plaques involving his whole body, which did not respond to antiallergy medication. He underwent a bone marrow biopsy and subsequent splenectomy. The bone marrow biopsy sample showed round uniform cells that were more densely packed along the paratrabecular areas (Figure 1). The cells had faintly eosinophilic to clear to slightly granular cytoplasm and a round nucleus containing coarse chromatin. The splenectomy material weighed 2250 g. The surface was irregular and had bosselated contours. The cut surface revealed nodules varying from 1 mm to 3 cm, which were slightly paler than the surrounding splenic tissue. Microscopically, there seemed to be a diffuse infiltrate most notably in the red pulp, but also involving the white pulp in varying degrees (Figure 2). This infiltrate was composed of cells similar in nature to those of the bone marrow. Few eosinophils could be identified within this infiltrate. Mitotic figures were inconspicuous. Sections of the accompanying hilar lymph node also showed effacement of the architecture by monotonous cellular infiltrate with granular cytoplasm and bland nuclei. In the subcapsular areas, the neoplastic cells showed a linear or singlefile arrangement (Figure 3). What is your diagnosis?
