Popis: | SUMMARY Sexually dysfunctional patients should not be pushed aside. If they cannot obtain a sympathetic hearing from their urologist, to whom can they go? An emotional block may produce as much flaccidity of the penis as a vascular or neurologic lesion. In addition to the usual physical examination, the differentiation between the functional and the organic causes of the sexual dysfunction requires a longitudinal history that documents the sequence of sexual development, the sequence of the development of sexual dysfunction, the patient's experience with various sexual partners, the onset and abatement of symptoms, and the presence or absence of nocturnal or morning erections. Even when the cause of sexual dysfunction is found to be a lesion or an organic disorder, the urologist has a responsibility to ascertain to what degree the lesion is contributing to the dysfunction and to what degree secondary anxiety, depression, and other psychologic factors may be contributing. A double-barreled therapeutic attack on both the organic and functional aspects of the problem will often succeed in patients in whom a single-focus effort will fail. The urologist needs to know a few principles of basic psychotherapy when treating the sexually dysfunctional patient. He must talk to and listen to both the patient and his sexual partner. If the urologist is unable to spend the few extra hours necessary, referral to an appropriate specialist, trained in treatment of sexual dysfunctions, may be necessary. |
