Although patient 1 was given lidocaine, and subsequently propranolol, the course was remarkably similar to that in patients 2 and 3, who received no treatment. By 2 to 3 months of age, all were free of the arrhythmia. On longer follow-up, none has shown signs of a recurrence. Because accelerated ventricular rhythm appears to be well tolerated in the newborn infant and most likely is self-limited, we believe that specific therapy is not indicated. A cautious approach in these children is recommended; however, because the long-term prognosis is still unknown, careful observation and occasional Holter electrocardiographic monitoring should be used to ascertain whether the child truly remains free of arrhythmia.
