In the United States, Type 2 Diabetes Mellitus (T2DM) has become an overwhelming healthcare burden and is the seventh leading cause of death. The Telehealth Visit (TV) is an effective tool for T2DM management. This project utilized an enhanced TV that included a standard TV plus additional elements incorporated from the Chronic Care Model (CCM). The purpose of this Quality Improvement (QI) project was to evaluate whether an enhanced TV increased the percentage of kept appointments and improved glycemic control (as measured by the percentage of Fasting Blood Glucose [FBG] in the range of 80-120 mg/dl) in adult T2DM patients over three months. This project was conducted at an outpatient endocrinology clinic. Inclusion criteria consisted of patients 18 years or older with an average HbA1c level greater than or equal to 7.5% over the previous year and were taking either oral diabetic medicine and/or insulin. Results revealed that only two out of 12 (16%) participants met the goal of average FBG level within the 80-120 mg/dl. One hundred percent of participants kept all their appointments. The weekly average FBG of the 12 participants decreased from 257 mg/dl in the first week and 134 mg/dl in the final week of the project. The majority of participants were strongly satisfied with TV. It is recommended that enhanced TVs be implemented for an extended period with a larger sample. |
