Popis: | We encountered 2 patients who underwent esoph-agogastroduodenoscopy for epigastric abdominal pain, whichshowed gastric mucosal erosions covered with adherent exudate.Microscopic examination of biopsies from the lesions obtainedfrom the 2 cases revealed characteristic pathologic abnormalitiesthat shared striking similarities. These included superficial mu-cosal necrosis and capillary vascular degenerative change in abackground of reactive or chemical gastropathy. Further reviewof records identified ongoing oral doxycycline use in both pa-tients. After cessation of the drug both patients’ symptoms re-solved. A follow-up esophagogastroduodenoscopy in 1 patientshowed normal gastric mucosa. This pattern of injury had notbeen previously described and seems to be characteristic fordoxycycline-induced gastric mucosal toxicity. Recognition of theclinical, endoscopic, and pathologic features described here mayfacilitate prompt diagnosis and management of this condition.Key Words: doxycycline, gastric ulcer, erosion, drug-induced,gastropathy, toxic-ischemic, vascular degeneration(Am J Surg Pathol 2013;37:259–263) |
