Insensitivity of a synthetic peptide-based test (Pepti-LAV 1-2) for the diagnosis of HIV infection in African children

Popis: Researchers used blood specimens from 3012 children at least 15 months old who were admitted to the pediatric wards of three university hospitals in Abidjan Cote dIvoire between late 1991 and mid-1992 to evaluate a synthetic peptide-based HIV test (Pepti-LAV 1-2). 245 children (8.1%) had tested positive for HIV-1 or HIV-2 on ELISA and Western blot. The Pepti-LAV 1-2 test had a sensitivity of 88.6% a specificity of 100% and positive and negative predictive values of 100% and 99% respectively. The Western blot test had detected gp41 (transmembrane glycoprotein) in 23 of the 28 specimens (82%) that yielded false negative results on the Pepti-LAV 1-2 test. The sensitivity of the Pepti-LAV 1-2 test was greater for children at least 24 months old than for those 15-24 months old (91.9% vs. 83.3%; p < 0.05). These findings suggest that this synthetic peptide-based test is less sensitive in younger children. A possible reason for the reduced sensitivity in children is that antibody titers increase with time and some children may have had antibody levels lower than the detection threshold of the Pepti-LAV 1-2 test.
