Electrocardiographic changes and importance of repolarization changes in cases with acute pancreatitis

Akut pankreatit hastalar›nda en s›k gorulen elektrokardiyogra- fi bulgusu literaturde belirsiz repolarizasyon deifliklikleri olarak bildirilmifl ise de son y›llarda bu repolarizasyon deifliklikleri- nin yuksek mortalite riski tafl›d›¤› bildirilmektedir. Bu cal›flmada akut pankreatit olgular›nda repolarizasyon deifliklikleri ile prognoz aras›ndaki iliflkiyi araflt›rmak istedik. Yontem ve Gerec: Gastroenterohepatoloji kliniine 2009 -2010 y›llar›nda yonlen- dirilen akut pankreatit tan›s› alm›fl olan hastalar cal›flmaya al›nd›. Butun hastalar›n ekokardiyografi, elektrokardiyografi, akci- ¤er grafisi, bat›n ultrasonografi ve/veya tomografi, Ranson ve Glasgow parametreleri ve biokimya testleri deerlendirildi. Bulgu- lar: Toplam 55 olgu (K/E: 22 (40%) / 33 (60%)) cal›flmaya al›nd›. Olgular›n 39'unda (70%) elektrokardiyografi deifliklikleri var- d›. Elektrokardiyografi deifliklikleri biliyer (%73) ve alkole bal› pankreatit (21%) olgularda daha s›kt›. En s›k gorulen elektrokar- diyografi deifliklii lateral erken repolarizasyondu. Lateral erken repolarizasyon ile Ranson skoru aras›nda anlaml› iliflki vard› (p = 0,005). Sonuc: Erken repolarizasyon akut pankreatit olgular›nda en s›k gorulen elektrokardiyografi bulgusu olup hastal›¤›n a¤›rl›¤› ile iliflkilidir. Background/aims: Acute pancreatitis is a systemic disease with high mortality. The most common electrocardiography finding in acute pancreatitis cases is reported to be nonspecific repolarization changes in the literature. Recently, it is reported that repolari- zation changes carry high mortality risk. In this study, we aimed to investigate the association between repolarization changes and prognosis in acute pancreatitis cases. Methods: Patients with acute pancreatitis referred to gastroenterohepatology clinic between 2009 and 2010 were included into the study. Echocardiography, electrocardiography, chest graph, abdominal sonography and/or computerized tomography, Ranson's and Glasgow's parameters and routine biochemistry tests of all patients were evaluated. Re- sults: A total of 55 cases (F/M: 22 (40%) / 33 (60%)) were included into the study. Thirty-nine of acute pancreatitis cases (70%) had electrocardiography changes. Electrocardiography changes were seen most frequently in cases with biliary (73%) and alcohol depen- dant pancreatitis cases (21%). The most frequently seen electrocardiographic finding was lateral early repolarization. There was a significant correlation between lateral early repolarization and Ranson score (p = 0.005). Conclusion: Early repolarization is the most common electrocardiographic findings and consistent with severity of acute pancreatitis cases.
