Popis: | Intrauterine contraceptive devices (IUCD) have been used for many decades. A number of complications including uterine perforation and migration into adjacent structures have been recorded. A 75-year-old women with an intrauterine contraceptive device (Lippes loop) that migrated from the uterus into the bladder over a period of 35 years with formation of a stone is reported. This is the longest period of migration that has been reported so far. A 74-year-old multigravid woman presented with a history of frequency urge incontinence and dysuria for 4 months. Physical examination revealed no abnormality. Urinalysis showed an abundance of leucocytes and significant growth of Escherichia coli. She was treated with an oral antibiotic (cotrimoxazole) and her symptoms improved. A Lippes loop had been inserted 35 years ago. Pelvic ultrasound revealed a mass located between the bladder and uterus with a free endometrial cavity. A plain radiography showed a large calcified mass encasing a Lippes loop (Fig. 1) in the pelvis. A cystoscopy revealed a 4.5 cm calculus in the bladder. (excerpt) |
