Síndrome de Parry Romberg

Popis: Introduccion : la hemiatrofia facial progresiva o sindrome de Parry Romberg es una enfermedad rara, de curso clinico lento, que genera atrofia de los tejidos blandos. Compromete frecuentemente un lado de la cara, siendo el izquierdo al parecer el mas afectado. La entidad se establece usualmente a edades tempranas y con el tiempo puede comprometer una o varias ramas del nervio trigemino. Se presenta un caso con diagnostico notoriamente tardio, pese a repetidas consultas por asimetria facial y otras manifestaciones. Caso clinico: paciente femenino de 73 anos de edad, natural y residente en area rural colombiana, con numerosas consultas por presentar asimetria facial, sin que le ofreciesen diagnostico definido. Antecedente de trauma cigomatico y del hombro derecho dos decadas antes. Dos cirugias oftalmologicas: una por cataratas y otra por glaucoma. Se observo depresion notoria en mejilla derecha con signos marcados de atrofia, acompanado de cefaleas migranosas periodicas y sintomas locales. Se realizo biopsia de piel donde se observo atrofia de los anexos cutaneos. No se demostraron otras lesiones neurologicas. Conclusion : el sindrome de Parry Romberg es una entidad cronica que puede tener implicaciones oftalmologicas o neurologicas. Involucra sintomas migranosos, parestesias y asimetria facial, que ameritan temprano diagnostico y tratamiento adecuado, tanto de la fase progresiva como estacionaria. SUMMARY Introduction: the progressive hemifacial atrophy or Parry-Romberg syndrome is a rare disease, of slow clinical evolution that generates atrophy of the soft tissues. It compromises frequently only one side of the face, where the left side is the most affected. The disease usually establishes in early ages and with the time it could compromise one or more branches of the trigeminal nerve. In this occasion, a case in which the diagnosis was notoriously late, despite repeated consultation due to facial asymmetry and other manifestations, is presented. Case record : a 73-year-old-female patient from a Colombian rural area, who attended several times to consultation due to facial asymmetry without obtaining a definitive diagnosis is presented. She had clinical history of orbito-zygomatic and right shoulder trauma two decades ago. Also, clinical history of two ophthalmological surgeries: Cataract and glaucoma. It was observed a notorious depression in the right cheek with marked signs of atrophy, accompanied of periodic migraine disorders and local symptoms. And a skin biopsy of the cutaneous adnexal, where the atrophy was observed, was taken. There were not demonstrated other neurological lesions. Conclusions: the Parry-Romberg syndrome is a chronic disease that could have ophthalmological or neurological implications. It involves migraine symptoms, paresthesias and facial asymmetry that merits early diagnosis and adequate treatment, so much of the progressive as stationary phase.
