Case 1: Acute Pancreatitis in a 13-year-old Girl

Helfrich, DO* 1. *Pediatric Clinic, 52nd Medical Operations Squadron, 52nd Medical Group, Spangdahlem Air Base, Germany A previously healthy 13-year-old girl presents to her pediatrician’s office for symptoms of diffuse abdominal pain, nausea, and persistent nonbloody, nonbilious vomiting several times daily for 4 days. She had 2 episodes of nonbloody diarrhea at the beginning of her illness but no diarrhea since. She was seen in the clinic 3 days earlier for similar symptoms and was diagnosed as having gastroenteritis, and she received a 1-L normal saline bolus and intravenous ondansetron. At that visit she passed an oral hydration challenge and was discharged from the clinic with home care advice. On her return, her pain has localized to her epigastrium and is described as sharp, persistent, 6 of 10 in severity, and nonradiating. At presentation she is afebrile, with a heart rate of 120 beats/min, respiratory rate of 18 breaths/min, and blood pressure of 112/82 mm Hg. Her weight is 47.2 kg, which is 2 kg less than she had been at her previous visit. On physical examination she is tired, ill-appearing, and uncomfortable, with delayed capillary refill and guarding and tenderness to palpation in her epigastrium. She has normal bowel sounds and no abdominal distention or masses. She is referred directly to the local emergency department for treatment of dehydration, vomiting, and suspected acute pancreatitis.
