Popis: Studies of changes in the ground substance of sarcoid lesions were made with the electron microscope in tissues from eleven patients with sarcoidosis of the following sites: lymph nodes, skin, and lungs. Comparison was made on the one hand between ultrastructural and histological findings in the sarcoid granuloma, and on the other hand, between our results and the ultrastructural definitions of certain substances, as found in the literature. In addition to uranyl and lead, special stains, namely P.T.A., KMnO4 and Ag, were of great help for identifying various changes in the ground substance. In so-called hyalin substance, two components were detectable: granular substance related to hyalin proper, and microfibrillar substance related to paralamyloid. A third type of change in the ground substance was compared with fibrinoid. Relationship of these substances with intracytoplasmic filaments found in the reticular, epithelioid, and giant cells was discussed.
