Popis: | Occasionally, while opera t ing on in t r ac ran ia l aneurysm, the aneurysma l sac m a y be t o rn inadve r t en t ly f rom the p a r e n t vessel. I f t he pa ren t vessel m u s t be cl ipped to cont ro l the r e su l t an t hemorrhage , grave neurologic sequelae resul t frequent ly . Obviously i t is mos t desirable to control the hemor rhage wi thou t sacrificing the involved ar tery . The prob lem is p robab ly mos t severe in cases involving aneurysms of the in te rna l carot id ar tery . Recent ly , the au thors were faced wi th such a s i tua t ion involv ing a n aneu rysm of the in te rna l carot id ar tery . We were able to control bleeding successfully by packing muscle be tween the to rn a r t e ry and the under ly ing sphenoid bone. Pos t opera t ive ang iography revealed a marked ly dimin ished flow of blood pas t the poin t of opera t ive in te rvent ion . Subsequen t repea ted angiography, however, revealed progressive re-opening of the vessel. |
