Popis: | Aorto-enteric fistula is a well recognised and catastrophic complication of aortic reconstruction occurring in about 1% of patients. I The aetiology is considered to be related to the intimate apposition of the upper suture line of the prosthetic graft to the duodenum, with continual pulsation causing pressure necrosis and thence fistulation. Lower suture line fistulae may also occur. Various techniques for the prevention of this complication have been described, including: the reflection of the duodenum to the right into an intraperitoneal position to prevent close contact; 2 interposition of either omentum or patches of expanded polytetrafluoroe thylene between duodenum and prosthesis; 3 or a "Z" incision to allow redundant sac advancement over the suture lines of the anastomosis, which has to be planned at the beginning of the repair. 4 |
