Adolescent female with abdominal distention

Popis: A 14-year-old girl presented to the Emergency Department (ED) with 2 weeks of abdominal swelling and epigastric pain. She had a history of obesity, but noted an additional 14-kg weight gain over the preceding 2 months. Abdominal examination revealed an obese female with mild abdominal distention. The abdomen was diffusely firm and nodular to palpation, with mild generalized tenderness. A pelvic examination was not performed. Laboratory testing including a complete blood count, serum creatinine, electrolyte panel, urinalysis, and urine human chorionic gonadotropin was unremarkable. Computed tomography of the abdomen and pelvis revealed a 28 28 18-cm cystic mass arising from the left ovary (Figures 1–4). Gynecology was consulted and the patient was scheduled for outpatient surgical resection. Exploratory laparotomy with resection of the abdominal mass and left salpingooophorectomy was performed several days after the ED visit. Pathology revealed a mucinous cystadenoma of ovarian origin. The patient had an uneventful postoperative course, and was discharged home on postoperative day 2 in good condition.
