Vildagliptin in The Management of Type 2 Diabetes Mellitus in Queen Elizabeth II Hospital: A Single-centre Experience

| Popis: | Introduction: In Queen Elizabeth II Hospital (QE2), Vildagliptin is often used as the second or third line oral hypoglycemic agent (OHA) in the management of type 2 diabetes mellitus (T2DM). Malaysian Clinical Practice Guidelines (CPG) for the Management of T2DM states that dipeptidyl peptidase-4 inhibitor (DPP4-I) such as Vildagliptin could reduce HbA1c by 0.5% - 0.8%. [1] This study described real-life use of Vildagliptin and reviewed the change in HbA1c post-Vildagliptin prescription in T2DM management in QE2. Methods: From January 2019 till December 2019, a retrospective case note review was carried out in QE2. Irretrievable case notes and patients with poor medication compliance were excluded. Results: As a whole, HbA1c measurements were improved significantly by a mean of -1.44% (SD±1.9) post-Vildagliptin prescription (f values: 0.01 – 0.017). The results suggested a reduction of HbA1c measurements postVildagliptin prescription in these groups of patients. Conclusion: The significance of the findings in this study was limited by the small sample size. Our findings implied that: ✓ The efficacy of Vildagliptin in HbA1c reduction in real-life practice was in-line with that stated in the Malaysian CPG for the management of T2DM. [1] ✓ Vildagliptin at renal-adjusted dose could reduce HbA1c effectively. ✓ Use of Vildagliptin with insulin could improve HbA1c at a greater extend compared to its use with other OHA. ✓ Most races of T2DM patients in QE2, Sabah responded well to Vildagliptin treatment in terms of HbA1c reduction. Nevertheless, a larger study is warranted to address the limitation.
