Tuberculosis in or Near the Major Salivary Glands: Report of 3 Cases

Popis: CASE 1A 67-year-old woman presented with a painless swellingoftherightretromandibularregion.Onclinicalexaminationthe mass, measuring 2 3 cm, was found to be mobile,hard,andelastic.Thepatient’smedicalhistorywasunevent-ful. Ultrasound examination and a computed tomography(CT) scan of the parotid showed a well-demarcated enlarge-ment located in the superficial lobe of the parotid (Fig 1),whereas the chest radiograph was normal. The clinicaldiagnosis was that of a “mixed tumor” of the parotid, andsurgical resection of the lesion was decided upon. This wasaccomplished performing a superficial parotidectomy withsimultaneous dissection and preservation of the peripheralbranches of the facial nerve. The histological examinationshowed a tuberculous infection of an intraglandular parotidlymph node with a simultaneous involvement of the neigh-boring parotid gland parenchyma. For that reason a Man-toux test was then performed, which was positive. There-fore, after 2 days in our Department the patient wastransferred to the Medical Department for further antituber-culous medical therapy.CASE 2A 15-year-old girl was referred to our Department fromthe Medical Department due to a painful swelling of the leftpreauricular region, with a high clinical suspicion of tuber-culosis. A Mantoux test was positive and the patient wasprescribed antituberculous drugs. On clinical examinationthe mass, measuring 1.5 1.5 cm, was found to be mobilewith nontender and elastic consistency. The patient’s med-ical history was uneventful. A fine-needle aspiration biopsy(FNAB) of the mass was performed, showing intense inflam-mation. Ultrasound examination and CT scan showed a
