Popis: | A 65-year-old man presented with a 4 week history of weight loss, and intolerance to fl uids. After drinking a small amount of fl uid, he experienced a bubbling sensation, followed by coughing. 15 months earlier a centrally located nonsmall cell lung carcinoma cT4N3M0 was diagnosed. Patient was treated with chemotherapy, followed by radiotherapy. 8 months later there was haemoptysis and local tumour recurrence was seen during bronchoscopy. After four cycles of chemotherapy, local treatment with intraluminal brachytherapy (3.5 Gy) was started. A chest CT scan was obtained, which showed an air containing space connected with the left main bronchus (Figure 1). Bronchoscopy showed a hole at the brachytherapy site through which the mediastinum is visualised. Oesophagoscopy revealed the opening of the fi stula. A coated 12 cm stent was placed. After the procedure the fl uid intolerance disappeared and patient has been well since then. |
