[Variability between health areas in antibiotic consumption among pediatric outpatients of Principado de Asturias, Spain (2005-2018)]

Popis: espanolIntroduccion. Mas del 90% de las prescripciones de antibioticos en Espana se realizan en ambito extrahospitalario, con una alta variabilidad entre regiones sanitarias. El objetivo de este estudio es describir la variabilidad entre areas sanitarias del consumo extrahospitalario de antibioticos en la poblacion pediatrica asturiana entre 2005 y 2018. Material y metodos. Analisis comparativo y retrospectivo del consumo de antibacterianos de uso sistemico (grupo J01 de la clasificacion ATC, Anatomical Therapeutic Chemical Classification) en ambito extrahospitalario en la poblacion pediatrica de las ocho areas sanitarias del Principado de Asturias entre 2005 y 2018, expresado en numero de dosis diarias definida (DDD) por mil habitantes y dia (DHD). Resultados. El consumo medio de antibacterianos en la poblacion pediatrica asturiana (2005-2018) fue de 14 DHD (IC95% 13,4 – 14,6), con un aumento hasta 2009 (15,2 DHD) y descenso a partir de 2015 (11,9 DHD en 2018) y predominio del grupo β-lactamicos penicilinas (10,7 DHD). El consumo medio experimento diferencias estadisticamente significativas entre las areas de mayor y menor consumo (Aviles, 19 DHD; Oviedo, 11,5 DHD). Se observo una amplia variabilidad entre areas en el analisis temporal (diferencia de 11,8 DHD en 2011) y de grupos terapeuticos. El consumo de macrolidos en el area de Langreo y de quinolonas en el area de Jarrio fue 2,3 y 4,1 veces superior, respectivamente, que dichos consumos en el area de Gijon. Conclusiones. Existe una amplia variabilidad cuantitativa y cualitativa en el consumo extrahospitalario de antibioticos en la poblacion pediatrica de las diferentes areas sanitarias de Asturias. EnglishBackground. Antibiotics in Spain are mainly prescribed in the community sector, remarking a high variability between areas. The aim of this study is to describe the variability between health areas in the antibiotic consumption in the pediatric population of Asturias during 2005-2018 period. Methods. Retrospective and comparative study using data about consumption of antibacterial agents for systemic use (J01C group in ATC, Anatomical Therapeutic Chemical, classification) in the pediatric outpatients of the eight health areas of Principado de Asturias between 2005 and 2018, based on defined daily dose (DDD) per 1000 inhabitants and day (DID). Results. Mean antibiotic consumption in pediatric outpatients in Principado de Asturias (2005-2018) was 14 DID (CI95% 13.4 – 14.6). Consumption increased until 2009 (15.2 DID) and decreased from 2015 onwards (11.9 DID in 2018). β-lactam penicillins was the most consumed therapeutic group (10.7 DID). There were statistically significant differences among the areas with the highest and the lowest consumption rates (Aviles, 19 DID; Oviedo, 11.5 DID). A high variability between health areas was observed when analysing data temporally (difference of 11.8 DID in 2011) and therapeutic groups. The consumption of macrolides in Langreo health area and quinolones in Jarrio health area were 2.3 and 4.1 times higher, respectively, comparing to Gijon health area. Conclusions. There is a wide quantitative and qualitative variability of antibiotic consumption in the pediatric outpatients among the different health areas of Asturias.
