Popis: | JOHN D. BIRKHOFF, M.D . I : This was the first admission to Columbia-Presbyterian Medical Center for this forty-three-year-old white, married, male lawyer with a history of gross hematuria of eight days' duration . Two weeks prior to admission, the patient underwent excision of a right parotid tumor at another hospital which was diagnosed as a Warthin tumor . Surgery was uneventful, but during convalescence, gross hematuria developed, painless at first but later with intermittent left flank pain . An intravenous pyelogram (IVP) showed a left lower pole mass, and the patient was transferred to Columbia-Presbyterian Medical Center for evaluation. lie denied any prior urologic symptoms, abnormal urinalyses, or tuberculosis exposure. Of note in his history, was that he smoked 20 cigarets per day for twenty-five years . On admission, his temperature was normal, and examination of his skin and nodes was unremarkable . A healing scar was noted from excision of his right parotid tumor with a small salivary fistula . His abdominal examination revealed significant left flank and left upper quadrant abdominal tenderness without distention or peritoneal signs . His admission hematocrit was 38 and white blood cell count 11,000 . Urinalysis confirmed gross hematuria without pyuria ; his urine was sterile . Erythrocyte sedimentation rate (ESR) was 51 ,mo ./ |
