SCREENING FOR CANCER IN THE PATIENT WITH RHEUMATIC DISEASE

Popis: Every clinician should be able to discuss screening for cancer with their patients. This is not as easy or as straightforward as it might seem. The many published recommendations about cancer screening often conflict, and screening guidelines are sometimes based on incomplete or inadequate information. In order to provide appropriate counseling to patients and to make good clinical decisions about screening, it is necessary to understand what evidence supports—or contradicts—the standard recommendations for cancer screening. In addition, some patient populations, including patients with rheumatologic disease, have special risks for some types of cancer. These risks may alter the balance of risk and benefit for both screening and the evaluation of abnormal physical or laboratory findings. This article begins with a discussion of the types of cancer that are most often the target of cancer screening in general medical practice. Colorectal, prostate, cervical, and breast cancer screening guidelines are reviewed and some of the evidence (and controversy) behind these recommendations is discussed. A discussion of some of the special risks for cancer in patients with rheumatologic disease follows, and some modifications of standard screening guidelines are suggested for these high-risk patients.
