Testicular feminization syndrome with Sertoli cell adenoma

Popis: An 81-year old patient was presented in February 1980 with increasing abdominal swelling and distension. She had never menstruated and was sterile. She was happily married and had a normal sexual life. The breasts and external genitalia were atrophic corresponding to the patient's age. The pubic and axillary hair were scanty, but the overall habitus was this of an old normal-apperaring female. On physical examination an atrophic vagina was found which ended in a blind pouch without a cervix. A pelvic solid mass was noted and exploratory surgery was performed. At laparatomy there was no uterus, the left gonad was slightly enlarged and laid on the brim of the pelvis. The palpated tumor was predominantly on the right side of the pelvis although considerably protruding over the pelvis. There was no evidence of local spread or metastatic disease. No normal tubes and other adnexa were seen, nevertheless, a cord-like structure was observed on the left side. The tumor and the contralateral gonad with the cord were removed. The tumor weighed 8900 g and measured 16 by 11 by 6 em. It was encapsulated and it's surface was bossulated. The cut-surface showed central secondary cyst-formation and at places myxomatous degeneration. The tumor itself was white with a yellowish hue and had a gyrate appearance. On the cut-surface of the contralateral gonad a wellcircumscribed whitish nodule was observed (Fig. 1). First the left gonad was studied histologically.
