Popis: | 1. 1. A case of metastatic tumor to the ovary from the sigmoid is presented. The patient had signs and symptoms of androgen stimulation; morphologic evidence of progestagen stimulation was found in the uterus. 2. 2. The signs and symptoms of androgen stimulation regressed following surgical removal of the ovarian tumors and did not recur when subsequent widespread and fatal metastases developed. 3. 3. Chemical studies revealed a fall in urinary 17-ketosteroid excretion and a rise in pituitary gonadotropin measured as follicle-stimulating hormone following operation. These observations are consistent with the removal of an androgen-secreting tumor. 4. 4. The stroma of the tumor exhibited an intense theca-lutein reaction with pronounced sudanophilia. It is inferred that these luteinized cells were the source of both androgens and progestagens. Steps involved in biosynthesis of sex steroids are outlined and a possible mechanism to explain unusual patterns of hormone secretion suggested. 5. 5. The advisability of securing a urine specimen prior to operation for comparison with a postoperative specimen is stressed. |
