Therapeutic and toxic effects observed with different dosage programs of cyclophosphamide in treatment of steroid-responsive but frequently relapsing nephrotic syndrome

Popis: | Fourteen children with steroid-responsive but frequently relapsing nephrotic syndrome were treated with a combination of cyclophosphamide and prednisolone. In this controlled study, cyclophosphamide was given at two dosage levels, 2.5 mg. per kilogram per day for 90 days (low dosage) and 5.0 mg. per kilogram per day for 45 days (high dosage). Low-dosage treatment appears to be just as effective in inducing a sustained remission of the nephrotic syndrome as the same total amount of drug given in a higher daily dosage over a shorter period of time. However, the frequency of both unpleasant (alopecia) or potentially dangerous (severe leukopenia) side effects observed was higher with high-dosage than with low-dosage treatment. The low-dosage regimen appears to have the more desirable therapeutic-toxic ratio, since the disadvantages of high dosage outweigh the advantages. |
