Pulsatile tinnitus related to progestin from intrauterine device

Popis: ear that was synchronous with her heartbeat. The patientrelated the onset of symptoms to months after the birth ofher son. She denied head trauma, dizziness or hearing loss; experienced difficulty sleeping due to the intensity of tinnitus (score=8 on a visual analogue scale, VAS), and discreet evening swelling of her lower limbs. As to personal history, this patient denied systemic diseases as hypertension, diabetes, hyper- or hypothyroidism. patient had an intrauterine device (IUD) placed about 6 previously. physical examination, the patient was normotensive with normal bilateral otoscopy. She reported decreased during cervical compression maneuver over large onthe right, but with no change head rotation. auscultation revealed no thrills or murmurs withthe stethoscope. Laboratory workup: hemoglobin 14g/dL,hematocrit 41%, normal levels for total cholesterol and itsfractions, triglycerides and thyroid hormones. This patientexhibited normal pure tone and speech audiometry andimpedance tests.An imaging evaluation (Doppler ultrasound of carotidand vertebrals, tomography of temporal bones, magneticresonance of inner ears and cranial angioresonance) wasperformed, with no identified abnormalities (Fig. 1).After the most frequent causes of vascular PT wereruled out, it was hypothesized a relationship with progestin-containing intrauterine device.
