Psychoda albipennis'e bağli tekrarlayan üriner miyazis

Popis: | Myiasis is defined as the infestation of live human and vertebrate animals by larvae of varied species. There are more then 50 types of fly known to cause myiasis in humans in the tropical and subtropical countries. Myiasis is usually associated with poor health and hygiene, inactivity and ulcerating lesions. Human cases involving the genitourinary system are very rare and present with unusual symptoms. In this case, a patient is presented with recurring urinary myiasis caused by Psychoda albipennis from the Diptera order, which typically does not cause urogenital myiasis in humans. 29-year old woman with high socioeconomic status, living in an apartment in the city center was admitted to our hospital with repeated complaints of live maggots in the urine. The first two periods of gray, creamy-colored grubs live in patient's urine lasted for 15-20 days; The third period started 10 days before resorting to the hospital and still continues slowly decreasing in intensity. In these periods, the patient had abdominal pain, pelvic pain, abdominal bloating, frequent urination and urinary difficulties. The patient's laboratory tests were within normal values. Urologic and genital examination did not show any detectable features. The live larvae collected from urine samples were sent to investigate Veterinary Medicine. It was identified as the fourth stage of the moth fly Psychoda albipennis. The patient completely recovered after one week of urinary tract antiseptic, antibiotic usage and implementation of hygiene recommendations. No risk factor was identified in the patient who had proper hygienic conditions, was living in urban area and was of high socioeconomic status. The patient was questioned in detail for recurrent myiasis. It was learned that the continuous existence of small flies in the patient's bathroom. Flies were found to come from the opening of the apartment space ventilation. Disinfection of toilet, bathroom and environment was described in detail to the patient. After that the patient's complaints were completely disappeared. Myiasis infestation in our patient 6 months of follow-up was not seen again. It is thought that there are more cases than encountered in clinical urogenital myiasis; this case is presented to increase awareness of the public and the clinicians and to draw attention to the phenomenon. © 2018 Refik Saydam National Public Health Agency (RSNPHA).Miyazis canli insan ve vertebrali hayvanlarin, çeşitli sinek türlerinin larvalari tarafindan istilasi olarak tanimlanir. Tropikal ve subtropikal ülkelerde insanlarda miyazise neden olduğu bilinen sineklerin 50'den fazla tipi vardir. Miyazis, kötü hijyen ve sağlik koşullari, hareketsizlik ve ülsere lezyonlarla ilişkilidir. Insanlarda genitoüriner sistemi çok nadiren tutar ve alişilmadik semptomlar oluşur. Bu olğuda, insanlarda tipik olarak ürogenital miyazise neden olmayan Diptera takimina ait Psychoda albipennis'in etken oldugu tekrarlayan üriner miyazisli bir hasta sunulmuştur. Sosyoekonomik seviyesi yüksek, şehirde apartmanda yaşayan 29 yaşindaki kadin hasta tekrarlayan, idrarda gri-krem renkli canli kurtçuk düşürme şikayeti ile polikliniğimize başvurdu. Hastanin şikayetinin ilk iki periyodu 15-20'şer gün devam edip geçmiş; 10 gün önce başlayan son periyot ise azalarak halen devam ediyordu. Bu periyotlarda hastada karin ağrisi, pelvik ağri, karinda şişkinlik, sik idrara çikma, idrar yapmada zorlanma şikayetleri de mevcuttu. Hastanin laboratuvar tetkikleri normal değerlerde idi. ürolojik ve genital muayenelerinde de özellik saptanmadi. Idrardan toplanan canli larvalar araştirilmak üzere Veteriner Fakültesi'ne yollandi ve Psychoda albipennis 4. dönem larvasi olduğu teyit edildi. Hasta bir hafta üriner antiseptik ve antibiyotik kullanimi ile hijyen önerilerinin uygulanmasindan sonra tamamen iyileşti. Sosyoekonomik ve hijyen koşullari yüksek olan hastada başka risk faktörü tespit edilemedi. Tekrarlayan miyazis için hasta ayrintili olarak sorgulandi. Hastanin banyosuna, apartman boşluğuna açilan havalandirma penceresinden devamli küçük sineklerin geldiği öğrenildi. Hastaya banyo, tuvalet ve ortam dezenfeksiyonu anlatildi. Bunlarin yapilmasini takiben hastanin şikayetleri tamamen kayboldu. Alti aylik takiplerinde de miyazis enfestasyonu tekrar izlenmedi. Sanildiğindan ve kliniklerde karşilaşilandan daha fazla vaka olduğunu düsündügümüz ürogenital miyazis konusunda toplum ve klinisyen bilincinin artirilmasina dikkat çekmek amaciyla sunulmuş bir olgudur. © 2018 Refik Saydam National Public Health Agency (RSNPHA). |
