Combination chemotherapy of germ cell tumors of the testis with vincristine, adriamycin, cyclophosphamide, actinomycin D and medroxyprogesterone acetate

Popis: | 43 patients had histologically documented germ cell tumors with metastases which could no longer be treated by surgery or radiotherapy. Mean age of patients was 28 years range 2-60 years. 12 patients had received previous chemotherapy. The aim was to give as high doses of each drug (vincristine adriamycin cyclophosphamide actinomycin D and medroxyprogesterone acetate) as the patient could tolerate without severe side effects. A minimum leukocyte count of 1000-1500/mm was attempted. Patients were hospitalized during each 4-day course of therapy. The 12 nonresponders had a median survival of 23 weeks. The 31 responders had a median survival of 55 weeks. This was considered statistically significant (p 1/2 .005). Partial responders median survival was 42 weeks. Responses were seen in all histologic types except the 2 patients with choricocarcinoma and was most frequent with embryonal carcinoma 7 of 15. Patients with no prior chemotherapy or radiation had a higher response rate. Nausea and vomiting were universal. Total alopecia occurred in all. Intercurrent infections occurred in 11 but in only 2 was the infection life-threatening. A fall in hemoglobin was common. Stomatitis was severe in 15 patients. No deaths were related to drug toxicity. |
